A website it is actually an extension of your business
card- it's an online profile- it's an online brochure - or it's an
online sampling of your work. It gives you the capability of giving your
customer a significant amount of information. You want to entice
the customer to call and ask questions about your product or service.
Very few small businesses have the capability of getting a customer to
place an order without first talking to someone.
Ask any small business owner that has a "contact us" form that requires a customer to fill in data, how
much data they get? A popular answer will be NONE.

That doesn't mean that they don't exist, it just means
that most small businesses are the kind that exchange dialogue with the
customer during a transaction, so the reality is that having a page that
asks the customer to play secretary for you and type in their information, probably is not
going to work. Customers do that when they're buying from larger websites-
like Amazon and
eBay, where the purchase method is pretty much dictated by a
large, well established company.
